Physical Therapist (PT) - Summary
- Provide direct patient care, developing and implementing personalized physical therapy treatment plans.
- Monitor patient progress, adjust therapy as needed, and maintain accurate electronic documentation using advanced technology.
- Educate patients and their families on therapy goals, home exercise programs, and strategies for continued improvement.
- Collaborate with an interdisciplinary team to ensure comprehensive patient care and optimal outcomes.
- Uphold ethical standards and comply with all legal and regulatory requirements for physical therapy practice.
Duties & Responsibilities
- Conduct patient assessments to determine physical limitations and set measurable recovery goals.
- Design and execute individualized treatment plans based on patient needs and physician recommendations.
- Utilize manual therapy, therapeutic exercises, and modalities to promote functional mobility and pain reduction.
- Document patient evaluations, progress notes, and outcomes in an electronic health record system.

- Communicate with referring physicians and other healthcare providers to coordinate care plans.
- Participate in regular team meetings and contribute to the development of best practices in rehabilitation services.

Qualifications & Requirements
- Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program.
- Valid Physical Therapy License, or eligibility to obtain licensure as required.
- Successful completion of the National Physical Therapy Examination (NPTE) preferred.
- Strong interpersonal and communication skills to connect with a diverse patient population.
- Ability to use technology for documentation and patient progress tracking.
