Physical Therapist (PT) - Summary
- Deliver in-home physical therapy services for patients recovering from illness, injury, or surgery

Duties & Responsibilities
- Conduct comprehensive assessments of patient physical function, mobility, and safety within the home environment
- Develop individualized treatment plans and set measurable goals based on patient needs and physician recommendations
- Deliver hands-on therapy interventions to improve strength, endurance, balance, and mobility
- Educate patients and caregivers on safe mobility techniques, adaptive devices, and home exercise programs
- Coordinate with interdisciplinary teams, including nurses, occupational therapists, and speech-language pathologists, to ensure holistic care
- Document patient progress and maintain accurate clinical records in accordance with regulatory and company standards
- Communicate effectively with patients, families, and healthcare providers to facilitate continuity of care

- Participate in ongoing quality assessment and improvement initiatives
- Travel as required to provide therapy services to patients in their homes

Qualifications & Requirements
- Current and valid state licensure as a Physical Therapist (PT) required
- Graduation from an accredited physical therapy program
