- Document evaluations, treatment goals, progress notes, and maintain required prescriptions and signatures with accuracy and timeliness.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for the state of employment.
- Minimum of one (1) year of experience in a healthcare facility, demonstrating proficiency in physical therapy practices.
- Ability and willingness to travel within the assigned coverage area as required by patient needs.
- Possession of a dependable vehicle, a valid driver’s license, and current auto insurance.
- Ability to lift 50–100 lbs and adapt to irregular hours, as necessary to meet patient care requirements.
