Physical Therapist (PT) - Summary

Duties & Responsibilities
- Provide direct patient care, including assessments, treatments, and evaluations, in accordance with established standards and protocols.
- Develop individualized plans of care based on comprehensive assessments and patient goals.
- Monitor and document patient progress, adjusting care plans as necessary to achieve optimal functional outcomes.
- Collaborate with physicians, nurses, and support staff to coordinate comprehensive patient care and maximize therapeutic benefits.
- Maintain accurate and timely clinical documentation, including evaluations, treatment goals, progress notes, care plans, and physician orders.
- Provide guidance and direction to support staff, ensuring all aspects of care are delivered safely and effectively.
- Educate patients and caregivers on exercises, mobility strategies, and safety precautions to facilitate rehabilitation and prevent injury.
- Promote the value of physical therapy within the broader community and advocate for patient needs.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for the state of practice.
- Minimum of one (1) year experience in a healthcare setting, preferably home health or related field.
- Ability and willingness to travel within the assigned coverage area to provide patient care.
- Dependable vehicle, valid driver’s license, and current auto insurance required.
- Strong interpersonal and communication skills, with a patient-focused approach to care.
- Excellent time management, problem-solving, and organizational abilities.
