Physical Therapist (PT) - Summary
- Deliver high-quality physical therapy services to patients in a home health environment.

Duties & Responsibilities
- Provide direct patient care, evaluating functional needs, outcomes, and participating in collaborative consultations with other healthcare professionals.
- Develop, implement, and adjust individualized plans of care for patients based on comprehensive assessments and patient participation.
- Conduct therapy sessions in accordance with established standards of care and industry best practices.
- Supervise and direct support staff to achieve targeted patient outcomes and functional goals.
- Maintain accurate documentation, including evaluations, treatment goals, care plans, and all required records and signatures.
- Serve as a resource and leader within the community, promoting the benefits of physical therapy and advocating for patients’ well-being.
- Work in a physically demanding environment, including tasks such as lifting, handling, and transferring patients as required.
- Be available for irregular hours and participate in call coverage as needed to ensure continuous patient care.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license for the state of employment.
- A minimum of one (1) year experience in a healthcare facility, preferably in a home health or rehabilitation setting.
- Valid driver’s license, reliable transportation, and current auto insurance required for travel within the branch coverage area.
- Willingness to travel up to 50% within the assigned service area.
