Physical Therapist (PT) Job OverviewHillside Medical Group, a dedicated primary care provider located in San Antonio, TX, is seeking a full-time Physical Therapist (PT) to join its collaborative healthcare team. As a Physical Therapist (PT), you will play a vital role in helping patients recover from injuries, illnesses, or disabilities by developing and implementing effective therapeutic programs. This position is ideal for individuals passionate about delivering patient-centered care within an outpatient primary care setting while enjoying the vibrant community of San Antonio, Texas.
Responsibilities- Evaluate patients to determine functional limitations, impairments, and disabilities, conducting thorough assessments in line with physician orders.
- Establish individualized care plans, set realistic and achievable goals, and design comprehensive treatment programs tailored to each patient's unique needs and age group.
- Administer evidence-based physical therapy interventions to support patients in regaining mobility, minimizing pain, and improving quality of life following injury, illness, or other causes of impairment.
- Document patient progress, treatment outcomes, and modifications to therapy plans using electronic health record (EHR) systems.
- Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure continuity of care and optimal patient outcomes.
- Deliver outpatient physical therapy services as part of a multidisciplinary team in a primary care setting.
- Educate patients and their families on therapeutic exercises, home care programs, and strategies to prevent further injury or complications.
- Current, unrestricted Physical Therapist (PT) license issued by the State of Texas.
- Valid CPR certification.
- Proficiency in electronic health record (EHR) charting and documentation.
- Excellent written and interpersonal communication skills, with the ability to provide clear patient education and collaborate within a team.
- Demonstrated adaptability and flexibility in a fast-paced healthcare environment.
- Commitment to ethical practice and maintaining a clear, restriction-free professional license.
- Experience as a Physical Therapist (PT) is preferred but not required; new graduates are encouraged to apply.
