The Physical Therapist Assistant (PTA) works under the direction and supervision of a licensed Physical Therapist to implement care plans designed to restore function, relieve pain, and prevent disability for patients in a home health setting. You will be responsible for following established care plans, monitoring patient progress, and ensuring that therapy interventions are delivered safely and effectively. By collaborating with the broader healthcare team, you help to ensure that each patient receives individualized, compassionate care tailored to their unique needs.

Physical Therapist Assistant (PTA) - Summary
- Deliver direct physical therapy services to patients in their homes under the direction of a licensed Physical Therapist.
- Promote and uphold the philosophy and mission of Elara Caring.

- Serve as a key member of a collaborative and supportive healthcare team.
Duties & Responsibilities
- Implement physical therapy interventions as prescribed in the patient’s Plan of Care and certified by the physician.
- Follow the directives of the licensed Physical Therapist and report any changes in patient status promptly.
- Adhere to all State Practice Act Guidelines and company administrative policies to ensure the highest standards of care.
- Provide patient care as directed by physician orders, assessment data, and established standards of practice.
- Incorporate patient-specific goals into therapy sessions and monitor progress toward those goals.
- Educate patients and their families on the safe and effective use of assistive devices.
- Instruct patients and caregivers in home exercise programs and ensure they understand therapy recommendations for continued progress.
- Document all therapy interventions and patient responses accurately and in a timely manner.
- Participate in interdisciplinary team meetings and collaborate with other healthcare professionals to optimize patient outcomes.

Qualifications & Requirements
- Graduation from an accredited Physical Therapist Assistant (PTA) program approved by the American Physical Therapy Association.
- Current, unrestricted PTA license valid for practice in the state of employment.
- At least 6 months of experience as a Physical Therapist Assistant is preferred.
- Ability and willingness to travel within the designated coverage area as required by the position.
- Valid driver’s license, reliable transportation, and current auto insurance.
- Physical ability to lift 50–100 lbs and work in environments that may be physically demanding.
- Strong communication, organizational, and interpersonal skills.
- Commitment to maintaining patient confidentiality and upholding ethical standards of practice.
