Nurse Practitioner or Physician Assistant – Primary Care (NP/PA) Job Overview
We are seeking dedicated and skilled Nurse Practitioners (NP) or Physician Assistants (PA) to join the outpatient primary care team at a Federally Qualified Health Center (FQHC) in Northern California. This full-time or part-time opportunity allows mission-driven advanced practice providers to deliver high-quality care and make a significant impact within an underserved community. The NP/PA – Primary Care role is integral to our patient-centered model, and we welcome providers passionate about improving healthcare access for all.
Responsibilities
- Deliver comprehensive primary care services in an outpatient setting as part of a collaborative care team including physicians and other Advanced Practice Providers (NP/PA).
- Manage a daily patient volume of 18–20 individuals, focusing on preventive health, chronic disease management, and acute care needs.
- Provide patient education and promote health maintenance in alignment with FQHC standards and community needs.
- Coordinate with interdisciplinary team members to ensure holistic, high-quality care for all patients.
- Uphold documentation and reporting requirements in compliance with federal and organizational guidelines.
- Maintain a commitment to continuous professional development and evidence-based practice within the Nurse Practitioner or Physician Assistant scope.
- No call responsibilities required, supporting work-life balance for the NP/PA – Primary Care team.
Qualifications
- Current and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of California.
- Graduation from an accredited NP or PA program.
- Ability to work both independently and collaboratively within a multidisciplinary care team.
- Strong interpersonal, communication, and organizational skills to effectively serve a diverse patient population.
- Demonstrated commitment to serving underserved communities, with an understanding of the unique challenges and opportunities within a Federally Qualified Health Center setting.
Schedule
- Full-time Nurse Practitioner or Physician Assistant positions are preferred (40 hours per week, typically over 4 days).
- Part-time NP/PA opportunities are available (minimum of 30 hours per week).
