Nurse Practitioner (NP) – Telehealth Job OverviewFamily Medical Associates, P.C. is seeking a dedicated Nurse Practitioner (NP) to join our team in a telehealth capacity. This role provides the opportunity for full-time or part-time employment, delivering primary care services through remote audio and video consultations. By leveraging innovative telemedicine technologies, the Nurse Practitioner (NP) – Telehealth ensures that patients receive comprehensive care, including chronic disease management, urgent care, and medication management, all from the convenience of their own home or office.
Key Responsibilities- Conduct telehealth visits with patients using secure audio/video platforms, providing high-quality primary care services in a remote setting.
- Collaborate closely with onsite physicians to coordinate care plans, manage chronic conditions, address acute or urgent care needs, and oversee medication management.
- Maintain thorough documentation of all patient interactions, follow-up plans, and clinical recommendations in accordance with organizational policies and regulatory standards.
- Deliver patient education and guidance on preventive health, self-management of chronic conditions, and medication adherence during telehealth sessions.
- Participate in quality improvement initiatives and contribute to the continuous advancement of telemedicine best practices within the organization.
- Active and unrestricted Nurse Practitioner (NP) license in Massachusetts.
- Demonstrated ability to provide comprehensive primary care services in a remote or telehealth environment.
- Strong communication skills, with proficiency in conducting patient assessments and delivering care via virtual platforms.
- Comfort with electronic health record (EHR) systems and telemedicine technologies.
- Collaborative mindset, with a commitment to working as part of a multidisciplinary healthcare team.
- Previous experience in primary care, family medicine, or telehealth is advantageous but not required.
