Nurse Practitioner (NP) – Primary Care Job Overview
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our primary care team at a Federally Qualified Health Center (FQHC). This is a full-time position focused on delivering comprehensive outpatient primary care services. As part of an experienced and supportive clinical team, the Nurse Practitioner (NP) will play a vital role in providing high-quality patient-centered care to a diverse patient population.
Key Responsibilities
- Provide primary care services to patients in an outpatient setting, ensuring the highest standards of care and patient safety.
- Collaborate closely with physicians, nurses, and other healthcare professionals to develop and implement effective patient care plans.
- Utilize electronic medical records (EMR), specifically eClinicalWorks, to document patient visits, treatment plans, and follow-up care.
- Conduct comprehensive patient assessments, including medical histories, physical exams, and diagnostic testing as appropriate.
- Diagnose and manage acute and chronic health conditions, coordinating care and referrals as necessary.
- Educate patients and their families on preventive care, wellness, and management of chronic illnesses.
- Maintain a daily patient panel of approximately 18–20 individuals, ensuring timely and efficient patient flow.
- Participate in a rotating phone call schedule, approximately 3–4 weeks per year, with very light, phone-only responsibilities.
- Engage in quality improvement initiatives and uphold FQHC standards and best practices in primary care.
Qualifications
- Current, unrestricted license as a Nurse Practitioner (NP).
- Board certification as a Nurse Practitioner (NP) in Primary Care or Family Medicine.
- Experience in outpatient primary care or a similar clinical setting is preferred.
- Proficiency in using EMR systems, with experience in eClinicalWorks considered a plus.
- Strong clinical assessment, diagnostic, and patient management skills.
- Excellent communication and interpersonal abilities, with a commitment to patient-centered care.
- Ability to work effectively as part of a multidisciplinary healthcare team.
