Responsibilities
- Provide comprehensive outpatient primary care services to a dedicated patient panel, focusing on seniors and individuals with complex health needs.
- Manage a maximum panel size of 650 patients, ensuring personalized attention and continuity of care.
- Conduct an average of 10–12 patient visits per day, allowing for thorough assessments and quality interactions.
- Collaborate closely with a multidisciplinary care team, including Family Practice and Internal Medicine Physicians, RN Clinical Managers, and support staff, to deliver integrated care solutions.
- Perform in-office medical procedures such as suturing minor wounds, administering joint injections, conducting skin biopsies, delivering wound care, and performing preventive screenings relevant to geriatric patients.
- Utilize advanced, cloud-based electronic health records and integrated systems to enhance patient outcomes and streamline care delivery.
- Contribute to a culture of accountability by aligning care practices with established quality metrics and organizational values.
Qualifications
- Graduation from an accredited Nurse Practitioner (NP) program.
- Minimum of 2 years of outpatient primary care experience.
- Active, unrestricted NP license to practice in the state of Arizona.
- Demonstrated proficiency in in-office medical procedures, including minor wound closures, joint injections, skin biopsies, wound care, and preventive screenings.
- Strong collaborative skills and a patient-centered approach to care.
- Commitment to providing high-quality, comprehensive care for seniors, individuals with comorbid conditions, and underserved populations.
