Nurse Practitioner (NP) / Physician Assistant (PA) Job DescriptionWe are seeking a dedicated and certified Nurse Practitioner (NP) or Physician Assistant (PA) to join our established internal medicine and geriatric practice on a full-time basis. Our practice has been proudly serving the Northern New Jersey community since 1984, providing comprehensive primary care for adults. The Nurse Practitioner (NP) / Physician Assistant (PA) will work in a collaborative, patient-focused environment that emphasizes a respectful and supportive approach to healthcare. The position is full-time and based in Bayonne, NJ, offering an excellent opportunity to work in a region known for its vibrant community and accessibility.
Responsibilities- Conduct comprehensive patient assessments and develop individualized care plans tailored to each patient's unique health needs.
- Perform routine physical examinations and document medical histories accurately.
- Diagnose and treat both acute and chronic medical conditions, formulating individualized treatment protocols.
- Order, interpret, and follow up on diagnostic tests such as laboratory work and medical imaging in accordance with best practices.
- Prescribe appropriate medications and therapies as needed, ensuring compliance with legal and clinical guidelines.
- Utilize Electronic Health Record (EHR) systems to document patient information, care plans, and treatment outcomes.
- Collaborate effectively with physicians, nurses, and other healthcare professionals to optimize patient outcomes and ensure continuity of care.
- Educate patients and their families on health management strategies, preventative care, and available treatment options.
- Remain up-to-date with advancements in nursing and medical practices, as well as emerging healthcare technologies.
- Valid New Jersey license as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Demonstrated proficiency in patient assessment, diagnosis, and care management.
- Experience and knowledge of Electronic Health Record (EHR) systems.
- Basic Life Support (BLS) certification.
- Strong ability to work both independently and as an integral member of a healthcare team.
- Demonstrated commitment to working with diverse patient populations.
- Minimum of two years’ post-graduate experience in a primary care setting preferred; recent graduates with prior professional experience in a primary care role, such as a medical scribe or medical assistant (MA), will also be considered.
