Progressive Health of Delaware is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our innovative, lifestyle-focused primary care team. Key Responsibilities
- Deliver primary care services to a panel of patients, focusing on lifestyle medicine, preventive care, and chronic disease management.
- Conduct thorough assessments, develop treatment plans, and provide follow-up care, emphasizing strong patient-provider relationships.
- Maintain a manageable patient load of 10-15 visits per day, enabling you to dedicate ample time to each individual and provide truly comprehensive care.
- Utilize innovative care models, including Chronic Care Management (CCM) and Remote Patient Monitoring (RPM), to enhance patient outcomes.
- Collaborate with a multidisciplinary team, including Registered Nurses (RNs) and support staff, to streamline care delivery and administrative processes.
- Document patient interactions and manage healthcare records using EMR systems, ensuring accuracy and compliance with healthcare regulations.
- Participate in practice improvement initiatives that prioritize efficiency and minimize administrative burden, allowing for greater focus on patient care.
- Provide occasional telephone coverage, with minimal weekend call responsibilities throughout the year.
Qualifications
- Current and valid certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Experience in primary care, internal medicine, or family practice is preferred.
- Demonstrated interest in lifestyle medicine, preventive care, and patient-centered approaches to healthcare.
- Proficiency with Electronic Medical Record (EMR) systems and a willingness to adopt new, innovative care technologies.
- Strong communication skills and the ability to work effectively as part of a collaborative, interdisciplinary team.
- Active Delaware state license or eligibility to obtain licensure is preferred.
