Nurse Practitioner (NP) - Summary- Full-time primary care role focused on outpatient services for seniors with complex medical needs
- Patient panel size capped for improved continuity of care and manageable workload

- Manage a panel of up to 650 patients, seeing an average of 10-12 patients per day
- Lead and collaborate with a dedicated care team, including medical assistants, schedulers, and referral specialists
- Conduct office procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Participate in value-based care initiatives, including achieving patient satisfaction targets and clinical quality measures
- Ensure compliance with HEDIS and organizational care standards
- Maintain accurate and timely patient records using electronic medical systems
- Participate in shared call schedule based on provider team size
