Nurse Practitioner (NP) - Summary- Position: Nurse Practitioner (NP)

- Practice Setting: Outpatient, primary care clinics serving senior populations
- Care Model: Value-based care with a focus on patient outcomes and satisfaction
- Deliver high-quality, patient-centered primary care to a panel of approximately 650 seniors with complex, chronic conditions such as heart disease, diabetes, and hypertension.
- Manage an average daily patient load of 10–12, ensuring thorough and compassionate care for each individual.
- Work collaboratively with a dedicated care team consisting of a medical assistant, scheduler, referral specialist, and supportive leadership.
- Perform office-based procedures including suturing, joint injections, skin biopsies, and basic wound care as clinically indicated.
- Utilize value-based care strategies to improve patient outcomes, satisfaction, and reduce unnecessary hospital admissions.
- Participate in a rotating call schedule, with frequency determined by the total number of providers in the market.
- Document patient encounters and update electronic medical records accurately and promptly, ensuring compliance with regulatory standards.
- Promote a culture of respect, empathy, and excellence in every patient and team interaction.

- Active, unrestricted Nurse Practitioner (NP) license with current DEA registration.
- Minimum of two years of outpatient primary care experience, ideally serving adult or senior populations.
- Experience in geriatrics is a plus but not mandatory.
- Proficiency in performing office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Strong computer skills and familiarity with electronic health record (EHR) systems.
- Demonstrated commitment to value-based care, patient satisfaction, and continuous quality improvement.
- Ability to work effectively as part of an interdisciplinary care team.
