As a Nurse Practitioner (NP) at our Los Angeles, CA clinic, you’ll be at the forefront of a supportive, NP-led care model. Duties & Responsibilities
- Provide comprehensive primary care to adult and senior patients, utilizing best practices in preventive medicine and chronic disease management
- Coordinate and support care for highly complex dialysis patients (non-nephrology focused)
- Conduct and meticulously document Comprehensive Health Assessments and Health Risk Assessments, ensuring accurate clinical documentation and coding
- Administer diagnostic tests and preventive screenings as part of routine patient care
- Develop and implement personalized care plans tailored to individual patient needs
- Educate patients on disease management, preventive strategies, and overall health and wellness
- Collaborate with an integrated care team, including physicians, medical assistants, clinic managers, and other healthcare professionals, to optimize patient outcomes and ensure continuity of care
- Participate in quality improvement initiatives and contribute to a culture focused on delivering high-value, patient-centered care

Qualifications & Requirements
- Active Nurse Practitioner (NP) license in California
- Certification as a Nurse Practitioner (NP) from an accredited program
- Experience providing primary care to adult and senior populations preferred
- Strong clinical documentation and assessment skills, especially with Comprehensive Health Assessments and Health Risk Assessments
- Familiarity with value-based care models and patient-centered medical home concepts a plus
- Excellent interpersonal, communication, and teamwork abilities
