Nurse Practitioner (NP) Job Overview
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our mission-driven primary care team in Richmond, VA. Responsibilities
- Provide primary care services for a panel of senior patients, focusing on the management of chronic diseases and promoting holistic wellness.
- Conduct comprehensive assessments, develop individualized care plans, and collaborate with an interdisciplinary team to optimize health outcomes.
- Manage a patient panel of up to 450 seniors, with an average of 18 patients per day in an outpatient clinic setting.
- Utilize value-based care principles to deliver cost-effective and evidence-based healthcare services.
- Coordinate care with in-house consulting specialists in cardiology, podiatry, and other disciplines to address complex health needs.
- Engage patients in holistic health services, including acupuncture, tai chi, yoga, and nutrition counseling.
- Document all patient interactions, assessments, and care plans in the electronic medical record system in a timely and accurate manner.
- Participate in scheduled administrative time for care coordination, team meetings, and professional development.
- Work collaboratively with a dedicated care team, including a medical assistant and scheduler, to enhance the patient experience.
- Ensure patients have access to necessary services, including free door-to-door transportation as needed.
Qualifications
- Current and unrestricted Nurse Practitioner (NP) license.
- Board certification in Family Medicine or Geriatric Medicine is required.
- Experience working with senior populations and managing multiple chronic conditions is highly preferred.
- Strong commitment to providing compassionate, patient-centered care in a value-based care model.
- Excellent communication, organizational, and teamwork skills.
- Comfortable working in a fast-paced, full-time outpatient environment.

Work Schedule
- Monday through Friday, clinic hours from 7:30am to 5:00pm (last patient scheduled at 4:15pm).
- No night or weekend shifts required, supporting a healthy work-life balance.
