Nurse Practitioner (NP) Job Description
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our primary care team in Columbus, OH. This full-time position offers a rewarding opportunity for healthcare professionals passionate about providing high-quality, value-based outpatient care, particularly to senior patients with multiple chronic conditions. As a core member of a physician-led and mission-driven organization, the Nurse Practitioner (NP) will play a vital role in delivering holistic, patient-centered healthcare services aimed at improving the well-being and quality of life for our community's neediest populations.
Responsibilities
- Provide comprehensive primary care services to a panel of up to 450 patients, focusing on seniors managing 3-5 chronic diseases.
- Conduct outpatient clinical assessments, develop individualized care plans, and coordinate ongoing treatment for patients.
- Work collaboratively with a dedicated care team, including a medical assistant and scheduler, to ensure seamless and effective patient care.
- Utilize value-based care approaches to address complex health needs and enhance positive patient outcomes.
- Maintain a manageable patient workload, typically seeing 18 patients per day, with administrative time integrated into the schedule.
- Ensure all documentation, patient records, and care plans meet regulatory and organizational standards.
- Participate in a Monday through Friday clinic schedule, with clinic hours from 7:30 a.m. to 5:00 p.m., and no required nights or weekends, supporting work/life balance.
- Engage with in-house consulting specialists and leverage holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling to support comprehensive care.
- Coordinate with transportation services to facilitate patient access to care as needed.
Qualifications
- Current board certification in Family Medicine or Geriatric Medicine as a Nurse Practitioner (NP).
- Demonstrated experience and interest in working with senior patient populations and those managing chronic health conditions.
- Commitment to patient-centered, value-based, and holistic healthcare delivery.
- Strong communication, collaboration, and organizational skills.
- Ability to work effectively within a multidisciplinary team environment.
