Nurse Practitioner (NP) – Louisville, KY – Full-timeWe are seeking a dedicated Nurse Practitioner (NP) to join our mission-driven primary care organization in Louisville, KY. As a physician-led provider, our organization is committed to transforming the care of seniors by delivering affordable, high-quality, value-based healthcare. Responsibilities- Provide comprehensive primary care to a small patient panel of up to 450 seniors, focusing on managing 3-5 chronic conditions per patient in an outpatient setting.
- See an average of 18 patients per day, ensuring individualized attention and quality care.
- Work collaboratively with a dedicated care team, including medical assistants and schedulers, to deliver coordinated and holistic care.
- Participate in value-based care initiatives, optimizing patient outcomes and overall well-being.
- Utilize in-house consulting specialists, such as cardiologists and podiatrists, to support comprehensive treatment plans.
- Engage patients in holistic health services, including acupuncture, Tai Chi, yoga, and nutrition counseling.
- Maintain thorough documentation and administrative duties with built-in administrative time during your schedule.
- Uphold our organization’s mission and values by promoting patient-centered, respectful, and culturally competent care.
- Active Nurse Practitioner (NP) license in Kentucky.
- Board certification in Family Medicine or Geriatric Medicine required.
- Experience working with senior populations and chronic disease management is preferred.
- Strong commitment to value-based care and continuous improvement in patient outcomes.
- Excellent communication, teamwork, and organizational skills.
- Full-time position based in Louisville, KY.
- Monday through Friday clinic hours from 7:30 AM to 5:00 PM, with last patient scheduled at 4:15 PM.
