Nurse Practitioner (NP) Job Overview
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our primary care team in Louisville, KY. This full-time Nurse Practitioner (NP) opportunity offers a positive work-life balance and a supportive clinical environment that honors the healthcare needs of seniors with multiple chronic conditions.
Responsibilities
- Provide comprehensive, patient-centered primary care to a panel of up to 450 seniors, focusing on individuals with 3 to 5 chronic diseases.
- Conduct thorough patient assessments, develop personalized care plans, and manage ongoing treatment for chronic and acute illnesses in an outpatient setting.
- Work closely with an interdisciplinary care team, including a dedicated Medical Assistant and Scheduler, to coordinate care and ensure optimal patient outcomes.
- Utilize value-based care principles to improve patient health and reduce unnecessary hospitalizations and emergency room visits.
- Maintain accurate and up-to-date electronic medical records in compliance with organizational standards and regulatory requirements.
- Participate in regular administrative time included in your schedule, supporting quality improvement initiatives and professional development.
- Collaborate with in-house consulting specialists, including Cardiology and Podiatry, to provide holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling.
- Ensure continuity of care for patients, including overseeing health maintenance, preventive screenings, immunizations, and chronic condition management.
- Advocate for patient needs and coordinate door-to-door transportation services as needed to promote access to care.
Qualifications
- Board Certification in Family Medicine or Geriatric Medicine is required.
- Licensed as a Nurse Practitioner (NP) in the state of Kentucky, or eligible for licensure.
- Experience working with seniors and managing multiple chronic diseases is highly desirable.
- Strong commitment to value-based, patient-focused care.
- Excellent communication, collaboration, and organizational skills.
- Ability to work effectively as part of a multidisciplinary team in an outpatient clinic environment.
Schedule and Work Environment
- Full-time, Monday through Friday, with clinic hours from 7:30 AM to 5:00 PM (last patient scheduled at 4:15 PM).
- No nights or weekends required, supporting a healthy work-life balance.
- 100% outpatient care with a manageable daily patient volume (approximately 18 patients per day).
