Nurse Practitioner (NP) Job DescriptionPrimeHealth Clinic is seeking a dedicated and experienced Nurse Practitioner (NP) to join our team in a full-time capacity. As a primary care provider, the Nurse Practitioner (NP) will deliver high-quality healthcare to a diverse patient population and play a vital role in our community-based practice model. This position emphasizes both direct and indirect patient care, enabling the Nurse Practitioner (NP) to practice within a collaborative and supportive environment.
Responsibilities- Provide patient-centered care within a community-based primary care model, focusing on delivering high standards of service to all individuals.
- Carry and manage an independent panel of patients, ensuring continuity and comprehensive management of their health and wellness needs.
- Work closely with a consistent support staff team to optimize patient outcomes and clinic operations.
- Deliver approximately 70-80% direct patient care, which includes conducting patient visits, performing examinations, diagnosing conditions, and developing care plans.
- Manage 20-30% indirect patient care responsibilities, such as handling urgent messages, prescription refills, and coordinating urgent same-day or telehealth visits when necessary.
- Practice full-spectrum primary care, excluding obstetrics, and perform basic outpatient procedures as required.
- Average patient load is 14-17 patients per day, ensuring manageable caseloads with an emphasis on quality of care.
- Must possess valid Nurse Practitioner (NP) licensure.
- Ability to pass a background check is required.
- Preferred candidates will have at least 1-3 years of experience working as a Nurse Practitioner (NP) in a primary care or urgent care setting.
- California NP License is required.
- Experience in geriatrics and primary care is highly valued.
- Ability to reliably commute to or relocate to La Quinta, CA before starting work.
- Full-time employment with a standard schedule of Monday through Friday, 8:00 AM to 5:00 PM.
