Nurse Practitioner (NP) – Full-time Position OverviewPentaHealth is seeking a dedicated Nurse Practitioner (NP) to join our dynamic team at our outpatient practice. As a valued part of our five-physician group, you will play a crucial role in providing comprehensive patient care and supporting the health outcomes of our community. With a balanced approach to patient appointments, this full-time Nurse Practitioner (NP) position is ideal for professionals committed to value-based care and looking to make a significant impact.
Key Responsibilities- Conduct patient evaluations and manage acute and chronic conditions within an outpatient setting.
- Split schedule between walk-in appointments (approximately 50%) and ongoing panel management (approximately 50%).
- Collaborate with a team of five physicians and other healthcare professionals to ensure coordinated and high-quality care.
- Utilize Epic electronic medical record (EMR) system to accurately document patient encounters, treatments, and care plans.
- Educate patients and their families about medical conditions, treatment plans, preventive care, and wellness strategies.
- Participate in ongoing quality improvement initiatives and contribute to value-based care goals.
- Respond to walk-in patient needs with efficiency, professionalism, and compassion.
- Maintain compliance with all relevant healthcare regulations and best practices.
- Current licensure as a Nurse Practitioner (NP) with appropriate certification.
- Experience in an outpatient, family medicine, or primary care setting preferred.
- Proficient with EMR systems, especially Epic, or willingness to learn.
- Strong clinical assessment, diagnostic, and treatment skills.
- Excellent communication and interpersonal abilities to effectively collaborate with colleagues and engage with patients from diverse backgrounds.
- Commitment to evidence-based practice and continuous professional development.
