Key Responsibilities
- Lead and manage a primary care clinic team, including Primary Care Physicians (PCPs) and Nurse Practitioners (NPs).
- Oversee a small patient panel of 60-90 seniors, focusing on individuals with multiple chronic health conditions.
- Champion and implement value-based care models to improve health outcomes and patient satisfaction.
- Coordinate care with in-house consulting specialists, including cardiology and podiatry, as well as holistic health services such as acupuncture, tai chi, yoga, and nutrition counseling.
- Participate in ongoing Medical Director and Center Operations training, including PCP Black Belt Training, Leadership Pathways Program, and Medical Economics.
- Support center growth and patient engagement initiatives, collaborating with other clinical leaders to drive practice development.
- Ensure the highest standards of patient care, safety, and compliance with clinical protocols and regulatory requirements.
- Utilize analytics and Microsoft Office tools to monitor and improve center financial performance and clinical outcomes.
- Deliver care in a collaborative and inclusive environment, fostering professional development opportunities for team members.
- Provide local and occasional regional travel as required for operational and leadership meetings.
Qualifications
- Medical degree (MD or DO) in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- At least one year of clinical experience in a geriatric or family practice setting.
- Demonstrated leadership experience in a clinical environment is preferred.
- Strong communication, analytical, and leadership skills.
- Proficiency in Microsoft Office and the ability to utilize healthcare analytics.
- Ability to travel locally and occasionally regionally as required.
