Medical Director, Primary Care Position Overview
We are seeking a dedicated and experienced Medical Director, Primary Care (MD/DO) to lead a dynamic clinical team serving the senior population in Jacksonville, FL. This full-time role is an excellent opportunity for a board-certified or board-eligible physician with a passion for value-based care and leadership within a primary care setting. As the Medical Director, Primary Care, you will play a pivotal role in transforming healthcare delivery for patients with multiple chronic conditions, ensuring clinical excellence, and driving continuous improvement within the organization.
Key Responsibilities
- Lead and manage a multi-disciplinary team of Primary Care Physicians (PCPs) and Nurse Practitioners (NPs), fostering a collaborative and patient-centric environment.
- Oversee the delivery of high-quality, value-based care to a small panel of senior patients, typically managing between 60-90 individuals with three to five chronic diseases.
- Ensure clinical protocols, policies, and procedures are maintained and updated in accordance with best practices and regulatory requirements.
- Provide mentorship, training, and professional development opportunities to clinical staff, including participation in leadership programs such as PCP Black Belt Training and Medical Director Essentials.
- Monitor and analyze clinical outcomes, patient satisfaction, and operational performance to identify areas for quality improvement and growth.
- Collaborate with in-house consulting specialists in disciplines such as cardiology and podiatry, and support the integration of holistic health services including acupuncture, Tai Chi, yoga, and nutrition.
- Support the organization’s mission by actively participating in community outreach and patient engagement initiatives, including coordination of transportation services for patients as needed.
Qualifications
- MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or a related specialty from an accredited institution.
- Board Certification or Board Eligibility in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- At least one year of clinical experience in a geriatric or family practice setting; prior leadership experience is preferred.
- Demonstrated proficiency in leadership, communication, and analytical skills, with the ability to manage staff and drive organizational goals.
- Competency with Microsoft Office and the ability to travel locally and occasionally regionally as required.
