Medical Director, Primary Care Overview

The Medical Director, Primary Care will lead clinical teams and ensure excellence in the delivery of care to a senior patient population, with a focus on those managing multiple chronic conditions. Key Responsibilities
- Lead a multidisciplinary clinical team, including primary care physicians (PCPs) and nurse practitioners (NPs), to deliver exceptional care to a panel of senior patients.
- Manage small patient panels to ensure personalized and attentive care, typically ranging from 60 to 90 patients per provider.
- Oversee and support the implementation of value-based care models, promoting best practices in the management of chronic diseases common among geriatric populations.
- Facilitate ongoing training and development initiatives, including PCP Black Belt Training, PCP Essentials, and leadership programs for both physicians and operational staff.
- Monitor and drive clinical and financial performance, utilizing medical economics to inform decision-making and optimize outcomes for both patients and the organization.
- Collaborate with in-house consulting specialists such as cardiology and podiatry, as well as holistic health service providers including acupuncture, Tai Chi, yoga, and nutrition.
- Ensure seamless coordination of ancillary services, such as door-to-door transportation for patients who require assistance, enhancing access to care.
- Participate in the recruitment, mentoring, and evaluation of clinical team members, fostering a culture of professional growth and continuous improvement.
- Support clinic operations through effective communication, leadership, and adherence to organizational policies and procedures.
Qualifications
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) with specialized training in primary care, internal medicine, family medicine, geriatrics, or a closely related field.
- Minimum of one year of clinical experience in a geriatric or family practice setting, with a preference for candidates who have demonstrated leadership capabilities.
- Excellent leadership, communication, and analytical skills are required to effectively manage clinical teams and drive organizational performance.
- Proficiency with Microsoft Office applications and the ability to travel locally and occasionally regionally as part of leadership duties.
