Medical Director, Primary Care OverviewWe are a physician-led, mission-driven primary care organization committed to transforming care for the neediest populations, focusing on seniors with multiple chronic health conditions. We are seeking an experienced Medical Director, Primary Care to lead a clinical team and ensure the highest standard of care for our patients in a full-time role. Key Responsibilities- Oversee daily clinical operations, ensuring the delivery of high-quality, patient-centered care in a value-based care setting.
- Manage and support a team of Primary Care Physicians (PCPs) and Nurse Practitioners (NPs), fostering a collaborative environment.
- Supervise the care of a small, focused patient panel of 60-90 seniors with complex needs, primarily those with 3-5 chronic health conditions.
- Implement best practices and clinical protocols to optimize patient outcomes and support the organization’s mission.
- Participate in and complete leadership development programs, including PCP Black Belt Training, Medical Director essentials, and pathways for advanced leadership growth.
- Monitor and drive center financial performance, medical economics, and growth initiatives.
- Promote holistic, integrated health services, including collaboration with in-house specialists such as cardiology and podiatry, as well as holistic health offerings like acupuncture, tai chi, yoga, and nutrition.
- Ensure compliance with all relevant healthcare laws, standards, and policies.
- MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or a related specialty from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- At least one year of clinical experience in a geriatric or family practice setting is required; prior leadership experience is preferred.
- Demonstrated leadership, communication, and analytical skills.
- Proficiency in Microsoft Office and the ability to travel locally and regionally as needed.
- Participation in structured training programs focused on primary care excellence, medical economics, and center operations.

- The Medical Director, Primary Care role is full-time, supporting a Monday through Friday clinic schedule with patient visits concluding in the late afternoon.
