Medical Coder (HIM Coder Analyst, CPC, CCS) - Summary

Duties & Responsibilities
- Review patient health records for ambulatory surgery, observation, emergency department, and outpatient services
- Assign accurate ICD-10-CM and CPT-4 codes in accordance with official coding guidelines
- Ensure all coding practices are compliant with Medicare/Medicaid and other regulatory requirements
- Collaborate with physicians and Clinical Documentation Specialists to clarify documentation and resolve discrepancies
- Maintain a minimum coding accuracy rate of 95%, supporting high standards in clinical documentation integrity
- Work with healthcare providers and billing teams to support accurate charge capture and reimbursement
- Participate in ongoing training and professional development to stay updated on coding best practices

Qualifications & Requirements
- Minimum of 2 years of current, full-time hospital outpatient coding experience
- At least 2 years of ambulatory surgery coding experience
- At least 2 years of observation coding experience
- Proven experience assigning ICD-10-CM diagnosis codes
- Demonstrated ability to review and interpret medical records for coding accuracy
- Relevant coding certifications such as CPC (Certified Professional Coder), CCS (Certified Coding Specialist), or equivalent are highly preferred
