This full-time Medical Assistant (MA) / Chronic Care Manager (CCM) role is an excellent opportunity for healthcare professionals experienced in clinical procedures and interested in chronic care management. Key Responsibilities
- Assist healthcare providers with clinical procedures, patient examinations, and routine care tasks in a medical office environment.
- Prepare examination rooms, maintain cleanliness, and ensure all supplies and equipment are properly stocked and organized.
- Document patient information accurately, including vital signs, medical histories, and clinical notes, using electronic medical records (EMR) systems.
- Coordinate with the healthcare team to facilitate patient flow and maintain efficient office operations.
- Support chronic care management programs by helping patients manage long-term conditions and following established care protocols.
- Engage in ongoing training related to chronic care management to enhance professional skills and stay current with best practices.
- Communicate effectively and compassionately with patients, families, and team members to deliver a positive care experience.
- Adhere strictly to healthcare regulations, privacy policies, and organizational protocols to ensure patient safety and data security.
Qualifications
- Certification as a Medical Assistant (MA) or equivalent credential.
- Experience in clinical set-up and working with electronic medical records (EMR) systems.
- Previous exposure to chronic care management (CCM) is advantageous, but comprehensive training will be provided for motivated candidates.
- Strong attention to detail, organizational skills, and the ability to prioritize tasks in a fast-paced healthcare setting.
- Excellent interpersonal, verbal, and written communication skills.
