Licensed Practical Nurse (LPN) - Summary
The Licensed Practical Nurse (LPN) supports the delivery of comprehensive nursing care within a medical clinic, contributing to both in-person and telehealth patient services. This role involves collaborating with physicians, registered nurses, and interdisciplinary teams to ensure the highest quality of care and patient safety. As an LPN, you will leverage your clinical skills, attention to detail, and patient-centered approach to support the healthcare team and enhance patient experiences.
Duties & Responsibilities
- Provide individualized nursing care to patients in accordance with established policies and procedures, including assessment, implementation, and evaluation of care plans.
- Utilize telehealth platforms, including telephone and secure internet messaging, to coordinate and deliver nursing services, support patient follow-up, and facilitate communication between staff and patients.
- Administer prescribed medications (oral, topical, subcutaneous, intramuscular, and intravenous) and perform medical treatments as directed by healthcare providers.
- Recognize and respond to urgent or emergent patient care situations, collaborating with RNs and physicians to initiate appropriate interventions and emergency protocols when necessary.
- Assist with complex diagnostic procedures and specialized practices, preparing patients, handling specialized instruments, and monitoring patient conditions before, during, and after procedures.
- Educate patients and their families about healthcare topics, treatment plans, medication management, and healthy lifestyle practices, fostering a collaborative approach to care.
- Maintain accurate, timely, and detailed documentation of patient care activities, observations, and outcomes in compliance with clinical standards and regulatory requirements.
- Support the orientation and training of new staff and ancillary personnel, sharing knowledge of unit policies, procedures, and nursing standards of practice.
- Promote and uphold a healthy work environment that enhances patient healthcare delivery, staff satisfaction, and overall clinic efficiency.

Qualifications & Requirements
- Graduate from an accredited nursing program approved by the appropriate state agency and accredited by the Commission on Nursing Education Accreditation (CNEA).
- Current, full, active, and unrestricted LPN license as a practical or vocational nurse.
- Certification in Basic Life Support (BLS); additional certification equivalent to or including American Red Cross First Aid/CPR/AED is required.
- Minimum of 12 months hands-on patient care experience in an adult primary care outpatient setting within the last 24 months.
- Eligibility for and successful completion of a NACI background check.
- Must be a U.S. citizen.
