Licensed Practical Nurse (LPN) - Summary
- Provide high-quality nursing care within a multidisciplinary healthcare team.

Duties & Responsibilities
- Utilize telehealth tools, including telephone and secure internet messaging, to coordinate patient care and communicate effectively with patients and healthcare staff.
- Initiate, perform, and complete assigned patient care duties, adapting to the specific needs of varying patient populations.
- Assess patient conditions, determine appropriate procedures and treatments, and accurately document relevant patient information.
- Respond proactively to patient needs, including medication administration, equipment-based care, and patient or family education.
- Administer prescribed medications via oral, topical, subcutaneous, intramuscular, and intravenous routes in accordance with established protocols.
- Recognize and respond to urgent or emergent patient care situations, seeking assistance from registered nurses or physicians as necessary and initiating emergency interventions per guidelines.
- Support complex diagnostic tests and specialized procedures, which includes preparing patients, assisting with diagnostic examinations, handling specialized instruments or equipment, and monitoring patient status throughout procedures.
- Orient new staff and ancillary personnel to unit policies, procedures, and standards of practice, serving as a mentor and resource.
- Promote principles of a healthy work environment, enhancing healthcare delivery, patient and family education, and employee satisfaction.
- In the Dialysis Clinic, maintain dialysis and reverse osmosis machines and assist in the medical treatment of patients as required.

Qualifications & Requirements
- Graduate of an accredited nursing program (community college, junior college, college, university, or vocational nursing program) approved by the appropriate State agency and accredited by the Commission on Nursing Education Accreditation (CNEA).
- Current, full, active, and unrestricted license as a Practical or Vocational Nurse (LPN).
- Certification in Basic Life Support (BLS) and equivalent or American Red Cross First Aid/CPR/AED course.
- Minimum of 12 months of hands-on LPN patient care experience in an adult primary care outpatient setting within the last 24 months.
- United States citizenship.
- Ability to successfully complete a NACI background check.
