Licensed Practical Nurse (LPN) Position Overview
The Licensed Practical Nurse (LPN) plays a vital role in community health programs, providing direct patient care and clinical support within a dynamic healthcare setting. The LPN is responsible for triaging patients, whether they arrive by telephone or as walk-ins, determining the urgency and appropriate level of care needed. Working collaboratively with the Practice Manager, the LPN assists in the supervision of clinical support staff and ensures that all tasks are performed in accordance with the scope of their nursing license. This full-time position is integral to supporting the organization's mission, vision, and values, and requires adherence to established compliance protocols, as well as company policies and procedures.
Key Responsibilities
- Triage patients efficiently, both by phone and in-person, to assess care needs and determine the urgency of medical conditions.
- Support patient care under the guidance of physicians, ensuring high-quality, compassionate service at every step.
- Assist the Practice Manager in the supervision and coordination of clinical support staff, helping to maintain a productive and collaborative work environment.
- Perform duties as assigned, consistent with the scope of practice for a Licensed Practical Nurse (LPN).
- Document patient interactions and care provided using electronic medical records (EMR), maintaining accuracy and confidentiality at all times.
- Adhere to all organizational compliance protocols and follow established policies and procedures to ensure quality care delivery.

Essential Skills and Qualifications
- Graduate of an accredited school of practical nursing.
- Must hold a current practical nurse license as required by the Commonwealth of Massachusetts.
- Minimum of three years’ experience in an ambulatory or outpatient healthcare setting is preferred.
- Proficiency in using electronic medical records (EMR) is required to ensure accurate patient documentation.
- Current CPR certification is considered a strong plus.
- Demonstrated ability to work independently and as part of a team within a fast-paced healthcare environment.
- Strong communication, organizational, and interpersonal skills.
