The Licensed Practical Nurse (LPN) plays a crucial role in delivering quality healthcare services to patients at Ammonoosuc Community Health Services. Key Responsibilities
- Serve as an integral member of the healthcare team, supporting care delivery and coordination with physicians and mid-level providers.
- Welcome, prepare, and assist patients for clinical visits, ensuring they are comfortable and informed throughout their experience.
- Collect and accurately record vital health data, including height, weight, blood pressure, pulse, temperature, respirations, medication histories, and allergies as indicated for each patient.
- Administer specified medications through oral, subcutaneous, or intramuscular routes, maintaining detailed and accurate documentation of times and dosages.
- Perform phlebotomy, preparing lab slips appropriately and ensuring proper specimen handling according to clinical protocols.
- Maintain a comprehensive log of laboratory tests ordered, follow up with the reference laboratory as needed, and communicate results to the healthcare team.
- Assist providers during simple clinical procedures, ensuring patient comfort and supporting procedural efficiency.
- Act as a chaperone during examinations or procedures when necessary to ensure patient safety and compliance with clinical standards.
- Support providers with patient follow-up regarding normal laboratory results, maintaining clear and timely communication with patients.
- Keep storage areas, laboratories, and examination rooms consistently stocked, safe, organized, and clean to promote a healthy and efficient work environment.
- Clean and sterilize clinical instruments and procedure packs daily, adhering to infection control protocols.
- Participate actively in staff meetings, contributing to the continuous improvement of healthcare services and team dynamics.
- Assist with special clinics, such as Well Child, Blood Pressure screening, and Immunization clinics, to address specific patient needs and promote community health initiatives.
- Demonstrate a commitment to ongoing professional development by pursuing continuing education opportunities relevant to the Licensed Practical Nurse (LPN) role.
- Complete and sign all clinical documentation accurately and in a timely manner, ensuring compliance with regulatory requirements and best practices.
- Perform all job duties within the scope of current licensure or certification, maintaining professionalism and ethical standards at all times.
- Other related duties, work hours, and site assignments at Ammonoosuc Community Health Services may be required as needed.

- Current, valid LPN license in good standing as required by state regulations.
