Licensed Clinical Social Worker (LCSW) Job SummaryThe Licensed Clinical Social Worker (LCSW) plays a crucial role in providing comprehensive support to individuals, families, and groups facing a variety of medical, emotional, and social challenges. This full-time position at Olly Neal Community Health Center is dedicated to helping clients cope with chronic, acute, or terminal illnesses through a combination of therapeutic interventions, case management, and collaboration with other healthcare professionals. Responsibilities- Collaborate closely with medical professionals to evaluate patients' medical and physical conditions, assessing client needs for appropriate social work interventions.
- Refer patients to relevant community resources, programs, and support services to assist in their recovery and ongoing care.
- Investigate cases involving child abuse and take authorized protective action when necessary, ensuring the safety and well-being of vulnerable populations.
- Provide counseling to patients to help them overcome dependencies, recover from illness, and adjust to significant life changes.
- Monitor, evaluate, and document client progress using measurable goals as outlined in treatment and care plans, ensuring ongoing improvement and accountability.
- Adjust and modify client treatment plans to reflect changes in client status or evolving needs, maintaining flexible and patient-centered care.
- Organize and facilitate support groups, as well as counsel family members, to help them understand, cope with, and support the patient throughout the recovery process.
- Attend and actively participate in staff development programs, department-related conferences, and administrative meetings to stay current with best practices and enhance professional growth.
- Perform other related duties as assigned; the company reserves the right to add or change responsibilities at any time.
- Master's degree in Social Work or a related field from an accredited institution.
- Current and valid Licensed Clinical Social Worker (LCSW) certification is required.
- Preferred: At least five years of experience in a related clinical social work setting.
- Exceptional verbal and written communication skills to effectively interact with clients, families, and healthcare teams.
- Strong problem-solving abilities and critical thinking to address complex client concerns and develop effective care plans.
- Active listening skills to accurately assess client needs and respond with empathy and understanding.
- Social perceptiveness to be aware of and sensitive to the reactions of others in various situations.
- Coordination skills to manage multiple cases and collaborate with diverse professionals and community resources.
