Harrisonburg Community Health Center (HCHC) is seeking a dedicated Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to join our team. Role Overview
The Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) works collaboratively as part of the primary care team to identify, triage, and manage patients with behavioral health care needs. This full-time position integrates behavioral health services within the primary care setting, ensuring a holistic approach to patient wellness. Behavioral health clinicians provide counseling and support services for a wide range of mental health concerns, including depression, anxiety, trauma, chronic disease management, substance use, and family dynamics. Services are delivered to both adults and children, and include individual, group, and family counseling, as well as crisis intervention and care coordination.
Key Responsibilities
- Support primary health care providers in the identification and treatment of mental health disorders and psychosocial challenges.
- Conduct brief, consultative assessments and formulate initial diagnoses and treatment plans for patients referred by primary care providers.
- Deliver evidence-based individual and group counseling services to patients of all ages, within the scope of practice for an LCSW or LPC.
- Collaborate with the primary care team to manage and treat behavioral health issues efficiently and effectively.
- Facilitate interviews, evaluations, and mental health assessments for patients and their families.
- Provide crisis management, direct mental health services, and documentation as required, including care coordination and completion of necessary paperwork.
- Monitor and support clients with persistent mental illness, offering behavioral modification and supportive counseling to enhance psychosocial, environmental, and physical well-being.
- Offer educational resources and mental health materials to patients, caregivers, and staff.
- Assist with data collection and quality improvement initiatives for HCHC.
- Coordinate the integration of behavioral health and primary care services in collaboration with other providers and team members.
- Refer patients to specialty behavioral health care as appropriate, ensuring continuity of care.
- Participate in the provider team to monitor, organize, and analyze continuous performance improvement measures.
- Ensure patient dignity, confidentiality, and privacy, releasing information only according to federal and state regulations.
- Work with interpreter services to provide inclusive care for patients from diverse backgrounds.
- Perform other related responsibilities as assigned.
Required Skills and Abilities
- Demonstrated ability to prioritize care needs and apply strong cognitive reasoning skills.
- Awareness of diverse patient backgrounds and the impact of cultural differences on care delivery.
- Experience working within an integrated care model in a primary care setting is preferred.
- Comfort working closely with primary care medical providers in a consultative and collaborative style.
