The Licensed Clinical Social Worker (LCSW) is an integral member of the Behavioral Health Department, working in a collaborative, patient-centered environment. This full-time or part-time role involves providing counseling and psychotherapy to adults, children, adolescents, and their families. The LCSW works closely with a multidisciplinary team, including medical providers, to deliver integrated care services. The position is based onsite and follows a Monday through Friday schedule, supporting patients and their families to improve psychosocial functioning and overall well-being.
Responsibilities
- Provide individual, family, and group mental health counseling services to a diverse patient population using evidence-based therapeutic approaches.
- Facilitate small group counseling sessions, adapting best practices to meet patient needs and fostering a supportive therapeutic environment.
- Complete all required clinical documentation accurately and promptly, maintaining compliance with organizational policies and standards.
- Collaborate with the interdisciplinary team, including participation in meetings with both internal staff and external agencies to coordinate comprehensive care.
- Consult with primary care providers, psychiatrists, substance abuse counselors, and care coordinators to ensure holistic patient management.
- Make informed referrals by offering patients multiple care options based on clinical assessments and best practices.
- Maintain professional relationships with colleagues and referral sources, upholding a high standard of conduct.
- Engage in ongoing professional development and utilize current clinical and educational research to enhance the quality of therapy services provided.
- Adhere strictly to organizational policies and procedures, including those related to confidentiality and patient privacy.
- Assess, identify, and diagnose psychological, emotional, or behavioral issues using information gathered from interviews, tests, records, and consultations.
- Support medication management as prescribed by primary care providers, ensuring seamless integration with therapeutic interventions.
- Attend and actively participate in staff meetings and other required organizational gatherings.
- Perform general administrative and office duties as necessary to maintain efficient clinical operations.
- Complete and maintain all client-related paperwork, including federal and state-mandated forms, ensuring accuracy and timeliness.
- Provide crisis intervention, emotional support, and resource referrals to patients in need.
- Participate in case consultations with psychiatric consultants and communicate treatment recommendations to medical providers.
- Screen and assess patients for common mental health and substance use disorders, facilitating early intervention and appropriate care pathways.
- Facilitate patient engagement, follow-up care, and treatment adherence, supporting positive clinical outcomes.
- Utilize electronic health records (EHR) and tracking systems to document patient progress, treatment recommendations, and coordinate care with other providers.
- Adjust treatment plans for patients not progressing as expected, in collaboration with medical and psychiatric professionals.
- Develop relapse prevention and self-management plans for patients who have achieved their treatment goals and are preparing for discharge.
- Support both the mental and physical health needs of patients, working closely with medical providers and other mental health professionals.
- Facilitate referrals to evidence-based psychosocial treatments as clinically indicated, ensuring patients have access to comprehensive care options.
Qualifications
- Current and valid Delaware licensure as a Licensed Clinical Social Worker (LCSW).
- Strong knowledge of behavioral medicine and evidence-based therapeutic interventions.
- Demonstrated ability to work effectively as part of an interdisciplinary primary care team.
- Excellent written and verbal communication skills, with the ability to convey complex information clearly and compassionately.
- Solid understanding of psychopharmacology and its role in mental health treatment.
- Competence in designing and implementing clinical pathways and protocols for both individual and group therapy.
Working Conditions
This exempt position requires a minimum of 40 hours per week onsite. The role involves direct access to patient records, requiring strict adherence to professional standards of confidentiality and compliance with HIPAA regulations.
