Family Nurse Practitioner (FNP) - Summary- Provide high-quality, evidence-based primary care services across the lifespan
- Collaborate with multidisciplinary healthcare teams to improve patient outcomes
- Mentor, educate, and support other clinical staff and team members
- Champion preventive health, wellness, and patient education
- Contribute to Decypher’s commitment to excellence in healthcare services
- Diagnose and treat patients with acute and chronic health conditions within established protocols and clinical guidelines
- Manage minor trauma cases and promote recovery through individualized care plans
- Deliver preventive care services, including annual physicals and immunizations
- Educate patients and families on healthy lifestyle choices, disease prevention, and self-care strategies
- Lead and participate in staff education, in-service training, and case discussions
- Recognize and manage conditions requiring isolation, ensuring adherence to universal precautions
- Document patient care accurately and collaborate with other healthcare professionals to coordinate treatment plans
- Attend staff meetings to discuss cases, share knowledge, and contribute to ongoing quality improvement initiatives
- Perform other related duties as assigned by supervisors or management as needed

- Master's Degree in Nursing from an accredited academic program recognized by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
- Current, full, active, and unrestricted license as a Registered Nurse (RN)
- Current certifications in Advanced Cardiovascular Life Support (ACLS), Basic Life Support (BLS), and Pediatric Advanced Life Support (PALS)
- Ability to pass a National Agency Check with Inquiries (NACI) background check
- U.S. Citizenship required
- Experienced in family practice and primary care across diverse patient populations
- Skilled in patient assessment, diagnosis, and management of acute and chronic conditions
- Strong leadership abilities with a passion for staff education and mentoring
- Excellent communication and collaboration skills with multidisciplinary teams
- Demonstrates a commitment to continuous quality improvement and evidence-based practice
- This position is not authorized for telework; on-site presence is required
