Family Nurse Practitioner (FNP) Position Overview
CHP is seeking an experienced Family Nurse Practitioner (FNP) to join its dedicated team and contribute to community health programs. This full-time role is based in Berkshire County, Massachusetts, a region known for its natural beauty and vibrant community. The Family Nurse Practitioner (FNP) will deliver comprehensive primary and specialty nursing services, focusing on health assessments, disease prevention, and patient education. Key Responsibilities
- Conduct thorough health assessments, including patient histories, physical exams, and diagnostic evaluations, to identify health concerns and develop care strategies.
- Provide evidence-based primary care, including management of acute and chronic conditions, preventive screenings, and health promotion activities.
- Develop and implement individualized care plans in partnership with patients, focusing on optimizing health outcomes and supporting self-management.
- Promote healthy lifestyle choices and preventive measures, providing patient education on topics such as nutrition, exercise, disease prevention, and medication adherence.
- Collaborate with a multidisciplinary healthcare team to coordinate patient care, ensuring seamless communication and continuity of services.
- Utilize electronic medical records and point-of-care technology to document patient encounters in compliance with federal and state regulations, including HIPAA and OSHA.
- Maintain up-to-date knowledge of clinical best practices, relevant laws, and medical guidelines to ensure safe and effective patient care.
- Demonstrate excellent communication skills in interactions with patients, families, and colleagues, supporting clear and compassionate dialogue at all times.
- Meet productivity expectations, including seeing at least two patients per hour of patient-facing time.
Essential Qualifications
- Master’s degree from an accredited nursing program.
- Current licensure as a Family Nurse Practitioner (FNP) in the State of Massachusetts.
- Board certification as a Family Nurse Practitioner (FNP) by the American Nurses Credentialing Center (ANCC) or the American Association of Nurse Practitioners (AANP).
- Prescriptive authority as required by Massachusetts state law.
- Demonstrated experience providing primary care as a Family Nurse Practitioner, including familiarity with disease prevention and health promotion strategies.
- Proficiency in electronic health records and data management systems, with the ability to utilize technology effectively in clinical practice.
- Thorough understanding of all applicable federal and state healthcare regulations, including HIPAA and OSHA requirements.
- Strong written and verbal communication skills, with the ability to explain complex medical information in a clear and patient-friendly manner.
