As a Family Nurse Practitioner (FNP), you will play a vital role in providing comprehensive primary and specialty healthcare services to patients across the lifespan. This full-time position at Greenway Medical Associates is designed for a dedicated professional who is passionate about patient-centered care, health promotion, disease prevention, and chronic illness management. FNPs are integral members of the healthcare team, working collaboratively with physicians and other healthcare professionals to ensure high-quality, evidence-based care for individuals and families.
Key Responsibilities
- Deliver primary and specialty healthcare to patients of all ages, from pediatrics to geriatrics, ensuring a holistic approach to wellness and disease management.
- Collaborate with physicians, specialists, and other healthcare professionals to provide coordinated and integrated patient care.
- Conduct thorough patient assessments, including health histories, physical examinations, and evaluation of medical complaints.
- Diagnose acute and chronic health conditions, utilizing clinical judgment and evidence-based guidelines.
- Develop individualized treatment plans, prescribe medications, and monitor patient progress to achieve optimal health outcomes.
- Order, perform, and interpret diagnostic tests, such as laboratory work and imaging studies, to inform clinical decision-making.
- Educate patients and their families about health maintenance, disease prevention, and management of chronic conditions.
- Maintain accurate and up-to-date electronic health records in compliance with legal, ethical, and organizational standards.
- Advocate for patient needs and facilitate referrals to specialty care providers or community resources as appropriate.
Qualifications
- Current and valid license to practice as a Family Nurse Practitioner (FNP) in accordance with state regulations.
- Certification as a Family Nurse Practitioner (FNP-BC or FNP-C) from an accredited certifying body.
- Graduate of an accredited Master’s or Doctoral Nurse Practitioner program with a family practice focus.
- Demonstrated clinical experience in primary care, urgent care, or family practice settings.
- Strong interpersonal and communication skills, with the ability to foster effective working relationships with patients, families, and healthcare team members.
- Competency in electronic health record (EHR) systems and commitment to maintaining accurate patient documentation.
- Commitment to ongoing professional development and best practices in family medicine and healthcare delivery.
