Family Nurse Practitioner (FNP) Position Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our primary care outpatient team in Plant City, Florida. This is a full-time opportunity designed for a Nurse Practitioner who is passionate about providing high-quality healthcare services to patients across the lifespan, from young adults to seniors. As an integral member of our multidisciplinary team, you will play a pivotal role in delivering evidence-based medical care and fostering long-term patient relationships in a supportive and collaborative environment.
Responsibilities
- Deliver comprehensive primary care to patients 18 years and older in an outpatient setting, utilizing best practices to assess, diagnose, and manage acute and chronic health conditions.
- Conduct thorough patient examinations, develop treatment plans, and perform follow-up care with an average of 20 patients per day, ensuring each patient receives approximately 15-20 minutes of individualized attention.
- Maintain continuity of care by building strong, trust-based relationships with patients and their families, supporting their health and wellness across all stages of life.
- Participate in a call rotation, with the expectation of one week of call coverage annually.
- Collaborate with physicians, medical assistants, and administrative staff to ensure high standards of patient care and service delivery.
- Document patient encounters accurately and promptly in electronic health records, adhering to all regulatory and compliance guidelines.
- Engage in ongoing professional development and continuing medical education to remain current with the latest primary care guidelines and best practices.
Qualifications
- Current licensure as a Nurse Practitioner in the state of Florida.
- Experience or comfort in caring for adult and geriatric patient populations in a primary care or outpatient setting.
- Ability to communicate effectively with patients and families from diverse backgrounds; proficiency in Spanish is preferred but not required.
- Commitment to patient-centered care, teamwork, and continuous quality improvement.
