Family Nurse Practitioner (FNP) Job OverviewEast Texas Family Medicine is seeking a dedicated Family Nurse Practitioner (FNP) to join our dynamic healthcare team. As a Family Nurse Practitioner (FNP), you will play a vital role in delivering patient-centered healthcare services in a collaborative and supportive environment. This is a full-time opportunity located in Palestine, TX, designed for a compassionate and qualified FNP who is passionate about making a difference in primary care.
Key Responsibilities- Provide thorough patient assessments, including comprehensive physical examinations and detailed health histories, to patients across the lifespan.
- Diagnose and manage a variety of acute and chronic health conditions, ensuring quality care for pediatric, adult, and geriatric patients.
- Develop and implement individualized treatment plans tailored to each patient's unique needs and health goals.
- Educate patients and their families on health maintenance, disease prevention, medication management, and lifestyle modifications to promote long-term wellness.
- Document all patient interactions and medical information accurately using appropriate medical terminology and ICD-10 coding.
- Work collaboratively with physicians, nurses, and multidisciplinary healthcare teams to ensure coordinated and comprehensive patient care.
- Stay updated on the latest advancements and best practices in primary care, medical-surgical nursing, ophthalmology, and geriatrics to enhance patient outcomes.
- Master's degree in Nursing from an accredited educational institution.
- Current certification as a Family Nurse Practitioner (FNP).
- Valid state nursing license with prescriptive authority.
- Strong understanding of medical terminology and proficiency with ICD-10 coding.
- Experience and knowledge in managing geriatric patients, with sensitivity to their specific healthcare requirements.
- Excellent interpersonal and communication skills, with an ability to work effectively in a collaborative, team-based environment.
- Demonstrated commitment to delivering compassionate, patient-focused healthcare.
