Responsibilities

- Order, perform, and interpret diagnostic tests to aid in patient evaluation and care planning.

- Educate patients and their families about health maintenance, disease prevention, and management of medical conditions.
- Collaborate closely with physicians, specialists, nurses, and other healthcare team members to coordinate comprehensive patient care.
- Maintain accurate, timely, and confidential electronic medical records in compliance with regulatory standards.
Qualifications
- Active, valid, and unrestricted license as a Family Nurse Practitioner (FNP).
- Master's degree in Nursing with a specialization in Family Practice from an accredited institution.
- Minimum of one year of experience practicing as a Family Nurse Practitioner (FNP) preferred.
- Proficiency with electronic health record (EHR) systems for documentation and patient management.
- Experience in utilization review and case management processes.
- Strong communication and interpersonal skills for effective patient interaction and teamwork.
- Ability to work independently, exercise sound clinical judgment, and manage time efficiently.
- Proficiency in a second language is preferred and considered an asset.
Certifications and Requirements
- National Provider Identifier (NPI).
- Current credentials with Medicare and Medicaid programs.
