Family Nurse Practitioner (FNP) Position OverviewFamily Medicine at Greenhill is seeking a dedicated and experienced Family Nurse Practitioner (FNP) to join our team and provide comprehensive primary care services in Wilmington, Delaware. This full-time role offers the opportunity to care for a diverse panel of patients, ranging from newborns to seniors, in a supportive and collaborative practice environment. As an FNP, you will be responsible for delivering preventive care, managing acute and chronic conditions, and performing select office-based procedures to meet the needs of our patient community.

Key Responsibilities- Provide office-based primary care to a dedicated panel of patients of all ages, from infants to older adults, as a Family Nurse Practitioner (FNP).
- Deliver preventive health services, acute illness assessment and care, chronic disease management, and office-based procedures as indicated.
- Offer care to additional patients as needed, supporting team-based care delivery throughout the practice.
- Participate in extended hour scheduling, including early morning, evening, and occasional Saturday morning appointments to ensure patient access in Wilmington, Delaware.
- Potentially provide primary care services to homebound patients, based on practice needs and your experience and interests.
- Collaborate closely with physicians, social workers, nurses, medical assistants, clerical staff, scribes, and managers to provide coordinated, patient-centered care.
- Graduation from an accredited Nurse Practitioner program and active licensure to practice as a Family Nurse Practitioner (FNP) in Delaware.
- Current and unencumbered DEA and Delaware controlled substance license.
- Minimum of two years’ experience in a primary care or comparable clinical setting.
- Experience with electronic health record (EHR) systems.
- Home visit experience is a plus.
- Bilingual candidates are encouraged to apply.
- Strong written and interpersonal communication skills.
- Ability to collaborate effectively within a multidisciplinary team to provide quality patient care.
