Family Medicine Physician (Primary Care, FM, PCP) Job OverviewWe are seeking a dedicated Family Medicine Physician to join our full-time team at an employer-sponsored primary care clinic in South Bend, IN. As a Family Medicine Physician (Primary Care, FM, PCP), you will play a pivotal role in providing comprehensive, proactive, and relationship-centered care for a diverse patient population. This full-time opportunity is ideal for a physician who is passionate about primary care and committed to delivering high-quality, patient-focused services that improve overall health outcomes within the community.
Responsibilities- Deliver a full spectrum of family medicine and primary care services for patients ages 2 and older, including preventive care, acute visits, chronic disease management, and annual physicals.
- Build and maintain strong provider-patient relationships to support long-term health and wellness.
- Conduct well-women’s visits, routine procedures, onsite dispensing, and lab services as needed.
- Collaborate with a clinical support team including Nurse Practitioners (NP), Physician Assistants (PA), and Medical Assistants (MA) to ensure efficient, high-quality care.
- Serve as a collaborating physician for Advanced Practice Providers (APPs) within the clinic.
- Manage patient volume effectively, averaging approximately 10-12 patients per day to allow for comprehensive, individualized care.
- Promote preventive health strategies and provide education and coaching for chronic disease management and wellness.
- Document all patient interactions, diagnoses, treatments, and outcomes accurately and in a timely manner according to clinic standards.
- Participate in quality improvement initiatives and contribute to a positive work environment focused on clinical excellence.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Completion of an accredited residency program in Family Medicine (FM), Internal Medicine (IM), or Internal Medicine-Pediatrics (IM-Peds). Candidates with Internal Medicine and some Pediatric experience or Urgent Care background are welcome.
- Board certification or eligibility in Family Medicine, Internal Medicine, or Internal Medicine-Pediatrics.
- Current, unrestricted medical license and DEA certification (medical licensing and DEA fees fully reimbursed by employer).
- Commitment to high standards of clinical quality, service excellence, and patient-centered care.
- Excellent interpersonal, communication, and teamwork skills.
