Complex Care Case Manager (LCSW/LICSW/LMHC) Position OverviewThe Complex Care Case Manager (LCSW/LICSW/LMHC) plays a vital role within a Federally Qualified Health Center (FQHC), focusing on providing intensive, wraparound care coordination for members who are frequent users of emergency department services. This full-time position is ideal for professionals with a passion for community health, who are skilled at integrating behavioral, medical, and social supports to reduce avoidable admissions and emergency department visits. The Complex Care Case Manager (LCSW/LICSW/LMHC) operates both independently and as part of a multidisciplinary team, working collaboratively to address the underlying causes of high utilization and to improve patient outcomes.
Key Responsibilities- Provide comprehensive care coordination and case management to individuals with complex healthcare needs, with an emphasis on those with frequent emergency department usage.
- Develop, implement, and monitor individualized care plans in collaboration with medical, behavioral health, and social service providers.
- Engage with patients and their families to ensure understanding and adherence to care plans, facilitating access to necessary resources and supports.
- Integrate behavioral health, substance use, and social determinants of health (SDOH) into care planning to address holistic needs.
- Document all care coordination activities in compliance with regulatory and organizational standards.
- Participate actively in multidisciplinary team meetings, contributing to shared planning and decision-making.
- Utilize excellent organizational skills to manage caseloads effectively and track patient progress.
- Support the mission of providing high-quality, accessible healthcare to underserved communities.
- Master’s degree in Social Work, Mental Health Counseling, or a related Human Services field is required.
- Active, unrestricted license as an LCSW, LICSW, or LMHC in Massachusetts.
- Minimum of 3–5 years’ experience in care coordination, case management, or community health.
- Preferred experience working with high-utilizer populations, as well as familiarity with behavioral health, substance use, and SDOH.
- Certification in Case Management is preferred, but not required.
- Reliable transportation is required to fulfill job responsibilities.
